Obtain the carrier’s actual option

Give the operating airline the route, date and clinician-assessed need and ask about its current stretcher process. Availability can depend on aircraft, seating configuration, required staff and time allowed for review. A travel agent’s preliminary airfare cannot establish a bookable stretcher. A connection may require a second operator and a separate approval. Ask which medical form or certificate the actual carrier requires, but do not publish a universal lead time or guarantee approval. The clinician and airline make different decisions.

Price the support outside the cabin

Compare any airline-stated seating or stretcher charges with escort ticket and duty, medical equipment, sending hospital-to-airport road transport, destination airport-to-receiver transport and any required airport handover team. The flight may cover only the air segment; it does not include a receiving hospital. Ask who handles physical transfer between beds, road vehicle and aircraft, including whether airport access is authorised. A quote that omits one road leg can look cheaper while offering an incomplete journey. Specify exact endpoints on every proposal.

Understand revision exposure

If discharge changes, the carrier may need to move the configuration to another flight or reassess it. Ask how fare, stretcher, escort and road deposits are treated under the actual terms. A changed patient condition may alter equipment and staffing as well as the airline decision; do not reuse an old approval as if it covered a new plan. Insurer authorisation may apply to selected components only. Before payment, document which supplier and payer bears each change. The case owner should circulate one revised route to the sending and receiving teams.

Seat and equipment dependencies

A commercial flight stretcher arrangement may occupy multiple seats and require a medical escort, airline-approved equipment and advance review; the operating carrier determines the configuration. Ask for airline charges and clinical staff charges separately, as well as ground transfers and any return travel for staff. A quoted airfare for one passenger does not describe the stretcher cost. If the carrier declines the proposed medical arrangement, the earlier quote is not an alternative mode approval. Compare a newly accepted plan rather than extrapolating from a published fare.
