Medical repatriation from China to Canada

Sending-side release and first road leg

The China sending hospital must confirm discharge readiness and a named handover point. Ask which summary, medications and equipment instructions may be shared with the overseas receiver through authorised channels. Road pickup has its own timing and scope; it does not include airline medical clearance.

Actual flights and mode

List all operating carriers and connections between China and Canada. The treating clinician assesses whether the proposed seated flight, escort, oxygen or stretcher is medically suitable; every carrier decides its own acceptance. A dedicated air ambulance is a separately assessed option if a scheduled flight cannot meet the clinical or timing requirements. Do not assume a through-ticket transfers medical approval across carriers.

Route-specific handover

Canada’s domestic onward segment can be operated by a different airline. Capture every operating carrier and determine where customs, transfer of equipment and the final road pickup actually occur. An insurer authorising the trans-Pacific sector may not have authorised the domestic flight or receiving facility.

Country-specific receiving and timing decision

For China-to-Canada travel, specify the province and final care city before choosing the entry flight. An international landing in one province and a hospital in another may require domestic airline review and a separate airport assistance request. Confirm the Canadian receiver can accept after that connection and that the family has the last road transport arranged. A China discharge summary sent to a case manager does not itself establish a Canadian hospital appointment or insurer payment.

Arrival and receiver

Arrange the Canada airport-to-hospital, rehabilitation or home leg separately and obtain a named receiver. A Canadian origin or destination may require a domestic feeder in addition to the trans-Pacific flight; define who covers the extra airport and road handover and do not assume one approval covers codeshare sectors. A locally booked ambulance, receiving bed or family caregiver should be confirmed before China discharge, not after the aircraft has departed.

Acceptance before commitment

From China to Canada, ask for the province and whether the first Canadian airport is also the final medical destination. If a domestic sector remains, the airline, airport assistance and road provider may all change at that gateway. A treating-doctor travel assessment from China does not book an admission in Canada. Ask the receiving hospital or caregiver to acknowledge the complete onward plan, and check the insurer’s definition of evacuation destination.

Authorisation and disruption

A Canadian route may involve a provincial destination beyond the first international gateway. Obtain the province, receiving hospital or family care contact and final domestic sector before ordering road pickup. Government of Canada guidance advises checking medical evacuation coverage, including whether return to Canada or the nearest appropriate care is covered; this does not determine any individual claim or guarantee payment.

The document and timing checklist

Before booking, request a current discharge or medical summary from the authorised treating team, the operating carrier’s relevant medical form if required, and confirmation of receiving-side acceptance. A provisional bed or a clinician’s opinion is not an airline acceptance. Assign a case owner to record decisions and expiry of time-sensitive approvals.

Decision pathway

If the patient can travel seated and the carrier accepts the plan, compare escort and ordinary travel support. If sitting is not feasible, obtain a carrier-specific stretcher review. If stability, equipment or timing makes scheduled service unsuitable, request clinical and aviation feasibility assessment. Confirm Canada receiving before release.
