Fit to fly and airline medical clearance: who decides what?
A clinical opinion and an airline's decision to carry a passenger address different responsibilities. Neither a medical escort booking nor a ground transfer booking substitutes for the operating carrier's acceptance.
What ‘fit to fly’ means in planning
The treating doctor considers the traveller's clinical condition and whether additional support or restrictions should be documented for the proposed journey. The term is often used informally, but an individual clinical opinion is not a promise that an airline will carry the passenger.
What an airline decides
The operating carrier applies its own current rules to the itinerary, aircraft, requested support and supplied medical information. It may require review by its medical team, request more documents, accept with conditions or decline carriage. A codeshare itinerary can involve more than one operating carrier.
MEDIF and MEDA
MEDIF commonly refers to an airline medical information form that collects the proposed journey, needed assistance and clinician-provided details. MEDA is used in some airline processes for medical assistance or approval; it is not a universal document or a guarantee of acceptance. Obtain the current form and instructions from each operating carrier.
When to ask early
Recent hospitalisation or surgery, uncertain fitness to travel, supplemental oxygen, a portable oxygen concentrator, commercial stretcher, medical devices or professional medical accompaniment can prompt carrier-specific review. A wheelchair request alone is not necessarily a request for medical clearance. Clarify the actual trigger with the airline before making fixed plans.
Prepare a usable exchange
Share the proposed flight and connections, assistance and device requirements, and any clinician documentation that the carrier requests through its approved channel. Leave clinical content to the treating team. Keep a record of the carrier response and any conditions, then confirm the airport and ground segments separately.
