Treat the connection as a new transport decision
Confirm the operating airline for each flight, connection airport, terminal and whether through-checking or a change of carrier applies. The treating team should assess whether the patient can make the whole journey; a ticket does not establish fitness for a second flight.
Identify the landside or airside path
Ask the airline and airport whether the patient remains in a controlled transit area or must clear entry formalities, collect luggage and recheck. That determines whether a third-party local ground team can meet the patient at all.
Reconfirm assistance for the second carrier
Wheelchair service, stretcher approval and onboard equipment need the operating airline’s confirmation for each flight where applicable. Do not assume a service approved on the international leg carries over automatically to a different domestic operator.
Give the destination a realistic arrival window
The receiving institution should know the final domestic airport, connection risks and who can notify it about missed flights. If the connection fails or care needs change, coordinate a clinically appropriate revised plan through the responsible teams.
