Choose the recipient before completing the letter
A Customs question, airline carriage request and receiving-hospital handover may require different evidence. Ask the intended recipient what it needs before asking a clinician to sign. This resource supplies a working structure, not an official Chinese government form. Where an authority or airline requires its own form, complete that process separately. A translation helps readers understand the document but does not add approval or replace any requested certification.
Make identity, product and quantity agree
Use the traveller’s document name and date of birth, then list every medicine separately. Include generic and brand names, all active ingredients for combination products, strength, formulation, prescribed dose, route, frequency and total travel quantity. Explain the clinical reason only to the extent relevant and authorised. A letter saying “regular medication” is difficult to match to several unlabelled containers; the inventory, prescription and packaging should tell the same story.
Explain the travel-specific support needs
Ask the clinician to identify any necessary needles, syringes, cooling or associated device and the relevant handling instructions. Add intended travel dates and why the supply is needed during the journey. Do not insert a generic statement that the medicine is legal in all countries. Controlled-drug classification, quantity and permit questions remain with the appropriate authorities; this letter cannot establish entry permission for tramadol or any other controlled product.
Obtain a real clinical review and retain the version
The clinician should check the content, add their name, professional role, institution, contact details, signature and issue date, and use institutional formatting where appropriate. The patient or coordinator may prepare a draft but must not impersonate the clinician. If the medication or travel quantity changes, ask whether the letter and prescription need updating. Retain the reviewed version and identify any translation so the receiving team knows which document was actually signed.
